- The individual must reside in Santa Fe County; both citizens and noncitizens are eligible.
- The individual must meet income criteria—Many people without insurance qualify. See the eligibility chart on this website.
- The individual must be referred by a physician to SFPA
SERVICES:
SFPA provides access to:
- Specialist services (see list of providers)
- Diagnostic services (lab, X-ray, CT scan, MRI, etc.)
- Hospital services (inpatient and outpatient at CHRISTUS/St. Vincent Regional Medical Center)
SFPA does not cover:
- Emergency Department visits, prescription drugs, dental care, or vision care
- Services that have already been provided to the individual
SFPA also provides:
- Help enrolling in Medicaid, and other programs
- Information on other health programs such as prescription drug programs, County Indigent Fund
Note: For SFPA there is no cost or copayment for the patient.
HOW IT WORKS:
- The patient fills out an application form with the primary care provider
- The form is faxed by the provider with the referral to SFPA (474-4663)
- SFPA issues a SFPA blue ID card to the patient for a six month period
- SFPA assists patient with accessing specialty providers
- SFPA can also help patients with other programs for ongoing insurance coverage
