THE DIFFERENCE

As a physician-lead program to improve the health of Santa Fe County, it is our goal to maintain an integrated network of healthcare services, providing the full continuum of care for our underserved citizens. It is our belief that helping people in need to control advancing health problems will allow those same people to maintain their jobs and contributions to the community. SFPA is the provider of last resort for these individuals, many of whom have no where to turn. Currently, patients who qualify for the program can receive the following services through SFPA:

• Eligibility Screening
• Primary Care Services
• Specialty Care Services
• Limited Diagnostic and Ancillary Testing
• Hospital inpatient and Outpatient Services

SFPA strives to maintain the lowest possible overhead in administering the program.  Through our seed funding, SFPA has been able to grow a budget which can easily maintain the program’s current offerings. Our partners are extraordinary organizations and individuals who believe that there is a better way to offer healthcare in our community. As our funding streams grow, we hope to offer expanded services such as:

• Behavioral health care services
• Subsidized pharmaceuticals
• Care Management
• Transportation
