FOR THE PATIENT
The dilemma of health insurance coverage for low-income workers is a pervasive problem in American society today. With over 45 million in the United States without healthcare, the social and economic consequences of the uninsured have yet to be completely quantified. SFPA helps those who are helping themselves by providing care to working individuals during a time of financial difficulty. Our goal is to get patients through a challenging health event and get these same people back into the workforce with limited consequence.
Project Access means that financial barriers to care are significantly reduced or eliminated. Each patient is provided with a coordinated system of care where each service arranged by SFPA is available through community pledges of care and services. This is not a guarantee of services by any means and providers in SFPA are under no obligation to care for our enrollees. SFPA only is able to provide services to residents of Santa Fe County due to its Charter and requirements of our funding partners.
If you wish to find out your eligibility or enroll in the program, this can be done through calling La Familia Medical and Dental Center at 505.982.4425. The majority of providers in SFPA are specialty physicians and we do require that patients are evaluated by a primary care provider prior to being referred into a program specialist. You may also ask your physician about the program to see if they are currently a participating provider. In this case, the physician office can print enrollment forms directly from this site and submit them to Rio Primary care for enrollment.
